It may not have been a commercial flight, but the crash of a Polish government aircraft on April 10, 2010 has left nearly 100 dead, including the Polish president and several other high ranking Polish officials.

The aircraft was a Tupolev TU-154, which is a Russian built plane.

What happened?
97 people died when a Polish government airplane crashed on April 10, 2010 about a mile (2 Km) from the airport in Smolensk, Russia including the Polish president and several other high ranking Polish officials. The pilot was attempting to land in heavy fog.
Poland's military chief and the head of its central bank were also among those killed in the crash.
Also of note are the dozens of passengers on board who were part of a Polish delegation traveling to Russia to commemorate the 70th anniversary of the mass murder of Poles in the nearby Russian town of Katyn.

What was the cause / Who is to blame?
The crash is under investigation, but it looks like it may be pilot error as it seems that the pilot may have ignored advice to reroute to another airport and instead attempted to land in heavy fog.

Any further safety issues?
The aircraft was maintained but allegedly there were complaints about how old the plane was, and possible safety issues.
